Mandated Physicals Examinations:  Are required on all tenth grade students along with updated immunizations in order to be in compliance with Connecticut State law.  The  State of Connecticut HAR (blue) form must be completed by the physician and is to include all mandatory screenings.  Sports forms will not be accepted in leu of state mandated forms

Mandated Sports Examinations:  Are required on all students participating in sports.  The Oxford School District medical form must be completed by the physician  and is to include all mandatory screenings.

Mandatory Health Screenings: Shall be required by the Oxford Board of Education for all students in accordance with State of Connecticut guidelines. Parents requesting exclusion from screening must provide written documentation of screening performed by a physician. 

Vision: Grade 9 & 10         Audiometric ( Hearing ): Grade 10                 Postural (Scoliosis) : Grade 10

Medication Administration:  Connecticut State law requires a physician's written order and parent/guardian written permission for the administration of medications including over the counter medications  to students.  Please contact the Health Office to discuss your child's needs.

Health Forms:  All necessary forms are available on the Oxford High School website or in the Health Office.
